Please write the name of the child you wish to donate in the name of, or you may leave this blank.

You may also indicate where you would like the funds to be directed, such as:

• Family support fund — supporting essential needs such as diapers, clothing, and food for Lucy McCormick families who may be experiencing financial hardship

• Snoezelen Room update

• Outdoor garden update
